Q. Good Morning Dr. Vadodaria, Hope this finds you well. My wife who is originally from the UK had Evolution / Outline Dermal Fillers injected into her Nasolabial folds in 2001-2. Since 2004 she has been getting recurring inflammation in the same filler area. The course of action has been visit the local Surgeon/Plastic Surgeon who inserts an injection and sucks out all the foreign matter and puts her on a course of antibiotics. Initially this used to occur every 2 years. Since the last 2 years it has occurred every few months and the present situation is that the infection won’t go away and she is permanently on antibiotics. Our current plastic surgeon has informed us that a ” Granuloma” has formed in the area and the Filler is migrating. He has suggested surgical intervention as a permanent cure, if not he has recommended a plastic surgeon in Milan/Italy who does Laser Surgery as a possible avenue. Can you please concur if the above is correct. Will Laser surgery cure her problem. Are there any doctors in the UK who do laser surgery to clear the Granuloma, as we would prefer visiting the UK to Milan. Any advice is highly appreciated. Kind Regards

A. Thank you for your email. I do feel that there are very competent surgeons  to deal with such granulomas following dermal fillers. You do not need to travel to Europe to deal with that. You could also inquire about the plastic surgeons with special interest in Laser to help you further. You need to find out the exact nature of dermal filler used and find out whether an antidote is available to neutralise the filler or not. I hope this helps. With very best wishes,

Q. I am eighteen years of age and am currently considering rhinoplasty along with buccal fat pad removal. Firstly, I’m aware that you perform rhinoplasties, so if possible I would appreciate it if you could provide me with a rough estimate as to how much this procedure would cost. I understand that it’s not easy to assess the exact price without a consultation but I’m just looking for a rough guide at the moment. (I doubt it will help without a photograph, but I’m mainly looking for rhinoplasty wide bridge plastic surgery. Along with this, I would hope to alter the shape of the dorsum, reduce the depth of the alar grooves and the bulbosity of the alar lobules, and further define the tip of my nose. A. Thank you for your email. The initial consultation is directly with Mr. Vadodaria for a period of one hour. The involvment of a nurse will be only for the preanaesthetic assessment. The cost of initial consulation will be deducted from the total cost of the procedure at the time of booking the operation. We believe that this is an appropriate fee for Mr. Vadodaria’s professional time and covering the administrative cost. Q. my age is 26 year old but i have very small size of breast look like a 13 yr old girl ….how i can enlarge it…suggest any gel also i m from India………

A. Hello and many thanks for your question!Breast enlargement today is a a very standard procedure in cosmetic surgery but the choices for the patient can be bewildering. However, the big advantage is that surgery can now be much more tailored for the specific needs and wishes of each individual patient; one does need to be fully counselled, informed and guided so that the final decision, if it is for surgery, will consistently result in excellent outcomes and satisfaction for surgeon and patient.  I have come across patients with your specific concerns fairly frequently and with a very small bust I would normally focus on making any procedure as natural looking as possible as there is less breast tissue available to disguise the implant. Patients like yourself have little or no ptosis (droop) which means that a submuscular pocket is possible and provides additional tissue in front of the implant to protect and help disguise it.  For longevity and optimal contour I would tend to advise cohesive silicone gel as the most reliable with a textured, preferably anatomical (‘teardrop’) profile. I am experienced with foam covered implants which could be considered; they are reputed to have less risk of capsule contracture, the main ong term cause of problems in breast augmentation.  If you happen to be in the UK, we would of course be delighted to see you!Best wishes
